Premier Dental Van Services – The Premier dental van will be on the San Antonio Elementary School campus September 3rd-5th, to provide dental health services for KG through 5th grade students. If you would like your child to be seen at the Dental Van please complete the consent form. Children must have a completed consent form to be seen on the Premier Dental Van.
Paper consents will be available in the clinic, if needed.
